To ensure the highest clinical standards, all applicants will be asked to provide the following documentation:

• A copy of your current professional licensure or certification

• Proof of completion of at least 16 hours of oncology rehabilitation–specific continuing education within the past five years
  (Examples include certificates from oncology-focused courses, conference workshops, Physiological Oncology Rehabilitation Institute (PORi) or Klose Training oncology programs, oncology exercise certifications, or oncology massage training)

• For those wishing to be listed as Certified Lymphedema Therapists, documentation for certification along with evidence of at least 10 hours of lymphedema-specific continuing education completed within the past five years
